2024-2025 LESD Student Athlete Packet

Packet only needs to be completed once per school year.  Not required for tryouts, but must be completed before the first practice.

ALL middle student athletes MUST have a sports physical completed  by a licensed physician and provide proof of insurance PRIOR to trying out for any school-sanctioned sports.

Please complete the Sports Physical forms. 

ALL student-athletes must also have medical insurance before they will be eligible to play. If your student-athlete does not have medical coverage, it can be obtained for a low price through 1st Agency. Each student-athlete must submit proof of coverage before they will be eligible to play.

The Litchfield Elementary School District does not provide any type of health or accident insurance for injuries incurred by your child at school. As a service to students and their families, LESD79 is making available a student accident insurance plan for your child at a very nominal cost. The district offers this program because of trends in rising family health and dental insurance costs, increased deductibles, co-payments, or lack of health or dental insurance coverage.

REASONS TO PURCHASE THIS COVERAGE:

1. Deductible and co-pays in your health plan. Many health plans have increased the amount of out-of-pocket expenses.
2. No insurance.

This plan will provide benefits for medical expenses incurred because of an accident. If you have other insurance, benefits can be applied to your deductible or co-pays.

If you have no other insurance, this will become your primary accident plan.

Fall Sports Schedules can be found HERE.  Schedules may be changed at any time due to unforeseen circumstances.  

Once a student has made the team and the $50.00 fee has been entered into the InTouch system by the front office staff, families may pay by using their ParentVue login credentials to log into the InTouch Receipting system.  

Arizona Revised Statute (A.R.S.)15-342 and Governing Board Policy JQ - Student Fees, Fines and Charges allow the school district to charge student fees, fines and charges for:

• Any injury to the head needs to be evaluated by a physician.
• Any kind of forceful blow to the head or to the body that results in the head moving back and forth.
• Any change in the student’s behavior, thinking, or physical functioning.

Unless directed otherwise, athletes will need to be driven to and picked up at the various schools after every away game. Parents must provide transportation to the games and home from all games. If carpooling, please complete and return the "Permission to ride home from games with another teammate’s family" form in the athletic packet. 

Exceptions: Students at Title I schools (L. Thomas Heck/School 17 and Wigwam Creek Middle) are provided transportation TO games, but parents must still arrange transportation home afterward.
